What Are Aftercare Programs?
Aftercare programs are structured support systems designed to help individuals maintain their recovery after completing a primary treatment program at a rehab facility. These programs provide ongoing resources, guidance, and accountability to address the challenges that can arise during the transition back to everyday life.
Aftercare typically includes services such as counselling, support groups and relapse prevention education. The goal is a continuum of care that reinforces coping skills, builds a strong recovery community and reduces the risk of relapse — so that leaving treatment is a step, not a drop.
How Aftercare Helps Recovery
Aftercare programs sustain long-term recovery by providing the support and tools to navigate life beyond treatment. They reinforce the skills built during rehab while addressing the new challenges of everyday situations. Through regular counselling, peer support groups and access to recovery resources, aftercare builds accountability and a sense of community, and helps you identify and manage triggers as they appear. It bridges the gap between treatment and independent living, which is exactly where recovery is most often lost.

Six Weeks of Reintegration
The weeks after you leave are the ones that decide it. Structured weekly sessions, remotely through the app, with your primary counsellor and clinical team — so discharge is a step down rather than a cliff edge.
Lifetime Aftercare
Two sessions a week, virtually, through the same app you used for the reintegration weeks. Not six months, not a year — lifetime. There is no point at which you age out, and no review that decides you have had enough.
Membership That Never Expires
Finish treatment and you are a member for life, automatically, with no application to make. The counsellors, peers and staff who know your story stay reachable on your terms, including if you have a hard year and need them again.

What makes the difference here is not the list of services. It is that you come back to the same people — the counsellors who sat with you, and the peers who were in the room. You do not re-explain yourself to anybody.
And there is no expiry on it. No annual review, no reapplication, no quiet point at which we decide you have had your share. If you need us in year one or year nine, the door is the same door.

How do I Help a Loved One With Addiction?
Seeing a loved one battle addiction can be incredibly painful and overwhelming. You might feel powerless and uncertain about how to assist them. The initial step in helping your loved one is to engage in an open and honest dialogue about their addiction. Share your concerns, offer your support, but also establish boundaries and encourage them to seek professional assistance. It can be beneficial to explore treatment options with our team and have resources ready for your loved one.
Throughout this journey, it’s important to stay patient, understanding, and supportive—recovery takes time. With the right mindset and support, you can guide your loved one toward a healthier and happier life, free from addiction. What if I Relapse?
The first step is to evaluate whether you need to return to rehab. If the relapse was a one-time event and you’re committed to reassessing or modifying your recovery strategy, going back to an inpatient facility may not be necessary. This environment offers hands-on care and continuous monitoring. However, if you find yourself falling into a recurring pattern of substance misuse, it may indicate the need for re-engagement in a structured treatment program. If discussions about substance use come up, socializing with those who encourage drinking, or resorting to substances as a coping mechanism arises, it suggests a more serious issue that requires immediate attention.
When you re-enter treatment after a relapse, the main focus should be on reintegrating into daily life. Choosing a sober living environment for a few months after treatment can be an effective way to prevent relapse, as having accountability and structure can support you during this vulnerable period. Additionally, establishing an reintegration therapy plan for ongoing support after rehab is advantageous.
What does treatment cost?
Twelve Mile is a private facility. We don’t bill insurance directly, but we can provide receipts for reimbursement, and financing is available through Medicard (iFinance) so the cost can be spread over time. We’ll walk you through the details on your first call.
