Twelve Mile Recovery is committed to protecting the privacy and confidentiality of your personal health information, in accordance with Ontario’s Personal Health Information Protection Act (PHIPA) and Canada’s Personal Information Protection and Electronic Documents Act (PIPEDA).
How We Collect and Use Your Information
We collect personal health information only for purposes related to your care and treatment. Information is collected with your informed consent, unless legally exempted or in an emergency.
When We May Share Your Information
We will not disclose your personal health information without your express written consent, except under circumstances permitted by law:
- During a medical emergency, when consent cannot reasonably be obtained
- To comply with mandatory reporting obligations, such as suspected child abuse or neglect
- If there is a court order, warrant or subpoena requiring disclosure
- During an official audit or evaluation of Twelve Mile Recovery by an authorized body
- If a crime has occurred on Twelve Mile Recovery premises or involves its staff
- To another healthcare provider, where you have authorized this through a signed Release of Information
What You Can Ask of Us
You have the right to:
- Access and request copies of your personal health information
- Request corrections or amendments if you believe there are errors
- Withdraw consent for the collection, use or disclosure of your information at any time, subject to legal or contractual restrictions
- File a complaint with us or with a privacy regulator, without fear of reprisal
